C. Due to structural abnormalities
• Gastric surgery
• Jejunal diverticulosis
• Enterocolic fistula
• Extensive small bowel resection
• Stricture (e.g. Crohn’s disease)

D. Due to impaired immune functions
• Hypogammaglobulinemia
