CXR P/A view showing:
Heart is normal in transverse diameter
Straightening of left border of heart with fullness of pulmonary conus.
Double shadow in right border of heart.
Upper lobe veins are dilated, upper lobe diversion.
Examination of precordium:
On inspection- visible cardiac impulse in mitral area
Apex beat tapping in nature
Thrill- diastolic in apical area
First heart sound- loud, Second heart sound- normal
Mid diastolic murmur in mitral area- low pitched, localized, rough, rumbling, best heard with bell of stethoscope in left lateral position with breathing hold after expiration. May be preceded by an opening snap and followed by a presystolic accentuation.
