  1. CXR P/A view showing:
  • Heart is normal in transverse diameter
  • Straightening of left border of heart with fullness of pulmonary conus.
  • Double shadow in right border of heart.
  • Upper lobe veins are dilated, upper lobe diversion.
  1. Examination of precordium:
  • On inspection- visible cardiac impulse in mitral area
  • On palpation-
  • Apex beat tapping in nature
  • Thrill- diastolic in apical area
  • On auscultation-
  • First heart sound- loud, Second heart sound- normal
  • Mid diastolic murmur in mitral area- low pitched, localized, rough, rumbling, best heard with bell of stethoscope in left lateral position with breathing hold after expiration. May be preceded by an opening snap and followed by a presystolic accentuation.
  1. Other investigations are:
  • ECG
  • Echocardiogram with colour Doppler
  • Cardiac catheterization
  1. Complications:
  • Atrial fibrillation
  • Systemic embolism
  • Pulmonary hypertension with CCF
  • Pulmonary oedema
  • Haemoptysis
  • Pulmonary infarction
  • Dysphagia
  • Interstitial lung disease
