Cardiomegaly

  1. CXR P/A view showing: Heart is enlarged in transverse diameter, left cardiophrenic angle is obliterated, right one is acute ——- Cardiomegaly

  2. Causes :

    • Multiple valvular heart disease
    • Cardiomyopathy
    • Pericardial effusion
    • Biventricular failure
    • Extensive myocardial infarction
    • Hyperdynamic circulation- severe anaemia, pregnancy, thyrotoxicosis, arteriovenous fistula, Beri beri
    • Shunt anomaly- ASD, VSD, PDA

    3. Investigations:

      • ECG, Echocardiography with Colour Doppler, Thyroid function tests

    4. Treatment of acute LVF:

      • Propped up position
      • High flow oxygen inhalation
      • IV or sublingual glyceryl trinitrate
      • IV furosemide – 50-100 mg IV
      • IV opiates / Morphine
      • Inotropic agent in hypotensive patient
      • Continuous monitoring of cardiac rhythm, BP, pulse oxymetry.
